Care Services

What is Hospice? 

Hospice care is designed for individuals in the final stages of an incurable illness, focusing on ensuring comfort and helping them live their remaining days as fully as possible.

Hospice professionals do not aim to cure diseases; instead, they focus on managing symptoms to enhance the quality of life. This approach emphasizes pain relief, emotional support, and holistic care tailored to each patient's needs.

Moreover, hospice care values the involvement of family members and caregivers, encouraging them to participate in decisions that impact the patient’s care. This collaborative approach fosters a supportive environment, allowing families to feel connected and empowered during this critical time.

TYPES OF CARE

Routine Home Care visits are made wherever the patient calls home, whether a private residence, a personal care home, assisted living facility, or nursing home. Visit frequencies varies on a case-by-case basis providing physical, emotional and spiritual support to the patient, including monitoring pain, managing symptoms, addressing nutritional needs, watching for emotional issues, and offering support. Team members also teach the family and/or caregiver how to provide the best personal care to the patient.

Continuous Care is provided when the patient is in a crisis and symptom(s) cannot be managed with routine home care. The hospice team continuously provides care at the patient’s residence until the symptom(s) are managed.

Respite Care provides temporary relief to the caregiver by transferring the patient to a facility that is contracted with Peaceful Purpose Hospice of Georgia for a maximum of five days.

Inpatient Care begins when other efforts to manage symptoms have been ineffective and a higher level of care is deemed necessary by the hospice team and attending physician as authorized by Medicare, Medicaid or commercial insurance. Provided in a contracted inpatient facility, this level of care is designed for stabilization of acute needs and symptom management and the patient returning to the routine home setting.

Medicare Part A, Medicaid, and select commercial insurances.

You’ll find comfort in knowing your loved ones’ hospice care team is always close by, allowing for faster response times. We have a care team available to provide care 24 hours a day, seven days a week who answer phone calls day and night and can respond within minutes, if needed. 

Innovative Practices: Smaller agencies are often more agile and open to adopting innovative practices. This flexibility enables them to implement new ideas and approaches quickly, ensuring that patients receive the most effective and cutting-edge care in a supportive environment.

Respiratory Expert:

Dr. Anil Puri, MD is our Medical Director and a Pulmonologist. He specializes in pulmonary disease, has over 19 years of experience, and is board certified in Critical Care Medicine. Dr. Puri expertise includes treatment of respiratory conditions such as COPD (Chronic Obstructive Pulmonary Disease) and emphysema.

Pain Mangement Expert:

Dr. Richardson, is our associate medical director, is highly experienced and dedicated medical professional with specialized training in Geriatric and Hospice medicine. As a fellowship-trained expert in these fields, Dr. Richardson brings a wealth of knowledge and compassion to the care of older adults and those with serious, life-limiting illnesses.

In addition to geriatric and hospice medicine, Dr. Richardson has pursued extensive education in Pain management, demonstrating a commitment to advancing expertise and delivering comprehensive care. Over the past seven years, Dr. Richardson has successfully managed a robust pain medicine program, providing care to over 5,000 patients. This experience has honed Dr. Richardson's skills in addressing complex pain issues, ensuring that patients receive the highest quality of life possible.

Dr. Richardson is recognized for a patient-centered approach, combining clinical excellence with compassionate care. This blend of expertise and empathy makes Dr. Richardson a trusted and respected figure in the medical community.

 OUR TEAM

 

The hospice care team is composed of specially trained healthcare professionals dedicated to ensuring that patients’ final days are comfortable, peaceful, and dignified. This interdisciplinary team typically includes:

  • Physician: Oversees the medical care and pain management, ensuring that the patient's needs are met. Patient’s preferred physicians are invited to collaborate with our physician and to stay as involved as they wish in the care plan.

  • Nurse: Provides hands-on care, monitors health conditions, and offers support to both patients and families.  Our nurses are experienced listeners and will comfort the family while also teaching them how to take the best care of their loved one.

  • Hospice Aide: Assists with personal care tasks, helping patients maintain comfort and hygiene.

  • Social Worker: Offers emotional and psychosocial support, counseling, and resources for practical needs, addressing family dynamics and coping strategies.

  • Chaplain: Provides spiritual care, addressing the emotional and spiritual needs of patients and their families, regardless of their religious beliefs.

  • Volunteer: Offers companionship and support, helping to relieve caregivers and providing a listening ear.

  • Bereavement Specialist: Supports families through grief before and after the loss, helping them navigate the emotional challenges of saying goodbye. Hospice families will receive bereavement support up to 13 months after the death of a loved one, including consistent contact, grief education and one-on-one visits.

This compassionate team works collaboratively to create a holistic care experience that honors the individual’s wishes and promotes a sense of peace during a challenging time. Together, team members make sure that patients are as comfortable as possible, with pain management and the ability to maintain the highest possible quality of life.

For after hour needs, Peaceful Purpose Hospice provides 24-hour telephone access to skilled nurses who can answer questions, provide caregiver support over the phone, or dispatch for an in person visit, if needed.

It all started…

With a touching story of resilience and compassion. The decision to create a wish-granting program not only honors the memory of the loved one but also transforms a widow's grief into a source of hope for others facing similar challenges. By acknowledging her own feelings and experiences during hospice care, she wanted to create a supportive space for patients and families to find joy and fulfillment in their time together. The Three Wishes initiative can foster a sense of community and provide lasting memories, helping others navigate their journeys with a bit of light in their hearts.

Three Wishes is supported by proceeds from the THRIVE foundation. This initiative aims to remind individuals facing terminal diagnoses that it's never too late to dream and that wishes can indeed come true.

Living with a terminal illness can be challenging, but through Three Wishes, we strive to create joyful moments and lasting memories. Thanks to generous donations from our families served and local community, our team has organized themed birthday parties, bedroom makeovers, shopping trips to favorite stores followed by dinners at beloved restaurants, and even golfing outings.

By focusing on celebrating life and making cherished memories, Three Wishes transforms the end-of-life experience into something meaningful and uplifting. Our goal is to ensure that patients feel valued and supported, allowing them to create joyful moments with loved ones during this critical time.
